This event is designed to bring together leaders and experts in the healthcare industry to explore innovative strategies for delivering value-based care

Succeeding Together in Value-Based Care 

The UC Value-Based Care Conference — Succeeding Together in Value-Based Care — is a two-day, in-person event designed to bring together leaders from population health, ambulatory care, and technology from across UC Health. The conference will focus on aligning value-based care priorities, sharing best practices, and accelerating systemwide improvement.

Susan Dentzer

President and Chief Executive Officer, America's Physician Groups

Susan Dentzer is President and Chief Executive Officer of America’s Physician Groups (APG) (apg.org), the nonprofit organization representing more than 360 large physician groups focused on patient-centered, coordinated, and integrated health care that is accountable for both costs and quality. Dentzer is a highly respected health and health policy thought leader,  a frequent speaker and commentator, and an author of commentaries in Modern Healthcare, the New England Journal of Medicine Catalyst, the American Journal of Public Health, and other prominent publications.  She is also the editor and lead author of the book Health Care Without Walls: A Roadmap for Reinventing U.S. Health Care.

In a time of rapid change in health care, Susan Dentzer, President and CEO of America’s Physician Groups, will share insights on the challenges and opportunities shaping the future of value-based care, particularly in Medicare and Medicare Advantage.  As the CEO of the only national organization promoting value-based care in Medicare Advantage, and with deep policy expertise, Susan has a unique set of perspectives, insights, and forward-looking ideas that explain why both Medicare Advantage and innovative models in original Medicare can have such wide impact on beneficiaries, health care organizations, and the heath care being delivered to older adults and those with disabilities.

Learn about UCSF's strategic approach to managing assigned Medi-Cal patients. The presentation will highlight the adoption of the CalAIM Enhanced Care Management program to enhance services and improve care delivery for patient populations within the San Francisco Health Plan and Alameda Alliance for Health.​​
